CANDIDATE CONSENT FORM

I understand that in order to properly assess my application, and to better assist me in finding a suitable temporary assignment, contractor, subcontractor, or employment opportunity, Telescope Recruitment Incorporated. (collectively, “Telescope Recruitment”) will need to collect the following types of personal information about me prior to placing or seeking to place me in a temporary assignment or other employment opportunity: contact information, education records, employment history, professional training and certifications, references from third parties named by me, skill testing results, interview notes, background check results, my resume, and other information listed in the application form.

I also understand that Telescope Recruitment may request that I provide Telescope Recruitment with further personal information, including, without limitation, my Social Insurance Number (“SIN”), prior to Telescope Recruitment placing me in a temporary assignment, contractor, subcontractor, or employment opportunity. To the extent that Telescope Recruitment requests such further information, I understand that it is doing so for the purpose of placing me in a temporary assignment, contractor, subcontractor, or employment opportunity in an efficient and timely manner, and that my disclosure of such further information to Telescope Recruitment is optional. Notwithstanding the foregoing, I further understand that in the event I am successfully placed by Telescope Recruitment in a temporary assignment, contractor, subcontractor, or employment opportunity I will need to provide Telescope Recruitment with such further personal information as Telescope Recruitment is required to obtain from me pursuant to applicable laws and regulations including, without limitation, my SIN.

I consent to Telescope Recruitment’s collection, processing and retention of my personal information for the purposes of assisting me in finding suitable temporary assignment, contractor, subcontractor, or employment opportunities and, in the event I am successfully placed, administering my employment or contractor relationship with Telescope Recruitment. To the extent my personal information is to be collected from third parties, I confirm that I have authorized those third parties to share my personal information with Telescope Recruitment.

I hereby authorize Telescope Recruitment and its representatives (including its parent, affiliates, partners, and contractors) to make inquiries and investigations into my employment/contractor and educational history, professional training and certifications, professional memberships, if applicable, and other related matters, as may be necessary in arriving at a decision regarding my application, pursuant to applicable laws. I agree to provide copies of transcripts and certificates from the educational institutions and professional associations listed above. To the extent permitted by law, I hereby release employers, schools and other persons from all liability in responding to inquiries connected with my application, and I specifically authorize any schools, businesses, individuals, services or other entities listed by me in my application materials to communicate with Telescope Recruitment to disclose to Telescope Recruitment any and all relevant personal information with respect to my educational and professional accreditations, work performance and behaviour. I understand that Telescope Recruitment may also be requesting a consumer credit report for the purpose of assessing my application, when authorized by applicable laws.

I consent to the disclosure of my personal information to companies that offer assignments for which I may be a suitable candidate and to Telescope Recruitment and its representatives (including its parent, affiliates, partners, and contractors) who will assist in providing me with temporary assignments or employment opportunities, and when Telescope Recruitment is required or authorized to disclose my personal information pursuant to applicable laws and regulations.

I authorize Telescope Recruitment and its representatives to send me information and materials relating to staffing and assignments through electronic mail and other media.

I understand that: (a) my personal information may be retained in hard copy at the Telescope Recruitment office where I submit this consent and stored in electronic form on Telescope Recruitment's secure servers located in Canada or the U.S.A. and other countries in which case my information may be available to local government authorities under lawful orders and laws applicable in such foreign jurisdictions; (b) Telescope Recruitment will take reasonable measures to safeguard the security and confidentiality of my personal information; (c) if I choose not to provide my consent to the collection, use and disclosure of my personal information as outlined herein, Telescope Recruitment's ability to assist me may be limited by law; (d) I may withdraw my consent at any time by providing written notice to Telescope Recruitment; and (e) my personal information may be retained for as long as is required by law and to fulfill the purposes described above, after which time it will be destroyed or deleted.

While I understand I have the right to access and review my personal information and to request that it be corrected or amended, I acknowledge that such rights are subject to, and may be limited by, applicable laws and regulations.

I understand that I can obtain further information about the contents of this Candidate Consent Form as well as further information related to how Telescope Recruitment will collect, process, and store my personal information by contacting the Branch Manager in the office in which I registered.

TELESCOPE RECRUITMENT incorporated will be storing your data in Recruit CRM to manage your information and send your profile to companies to present you for potential job Opportunities.
Recruit CRM is a software system that helps recruitment firms manage their relationship with their clients. You can view Recruit CRMs privacy policy or GDPR commitment on our website.
Recruit CRM may store your data in a Data Centre inside or outside Europe.
You can ask your recruiter at any point to delete your data or simply ask them for an “Update resume link” which you can use to update your profile information/resume or simply delete your data from their system.

I fully understand the above consent statements and I am entering into them voluntarily, as certified by my signature below.
