Key Responsibilities:

Conduct comprehensive assessments and evaluations of patients with complex wound care needs.
Develop personalized care plans tailored to each patient's wound management requirements.
Perform advanced wound care interventions including, but not limited to, debridement, dressing changes, and use of negative pressure wound therapy.
Collaborate with interdisciplinary teams to ensure a holistic approach to patient care.
Educate patients and families about wound healing processes and preventive measures.
Document all clinical findings and care provided in accordance with company policies and regulatory requirements.
Stay updated on the latest advancements in wound care therapies and practices.
Participate in patient rounds and case discussions, providing insights into wound management strategies.

Essential Qualifications:

Current Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of California.
Wound care certification (Wound Certified) is mandatory.
Master's degree in Nursing, Physician Assistant studies, or a related field.
Strong clinical skills in wound care and management.
Excellent verbal and written communication skills.

Desired Experience:

At least 2 years of clinical experience in wound care management.
Experience in an interdisciplinary healthcare setting.
Familiarity with current wound care technologies and treatment modalities.
Previous experience in home health care, hospice, or palliative care is a plus.
