Job Responsibilities:

Conduct thorough assessments and evaluations of patients with complex wound care needs.
Develop and implement individualized care plans based on best practices and clinical guidelines.
Collaborate with a multidisciplinary team to provide comprehensive wound management.
Monitor patient progress and adjust treatment plans as necessary to ensure optimal outcomes.
Educate patients and their families on wound healing processes, treatment options, and prevention strategies.
Document patient care accurately and timely in compliance with healthcare regulations.
Participate in staff meetings, training sessions, and continuing education opportunities to enhance skills.
Stay current with the latest advancements in wound care and relevant technology.

Essential Qualifications:

Graduation from an accredited Nurse Practitioner (NP) or Physician Assistant (PA) program.
Current and valid NP/PA license in the state of practice.
Wound Care Certification is required.
Strong clinical assessment skills and the ability to work independently.
Excellent communication and interpersonal skills.
Strong organizational skills and attention to detail.

Desired Experience:

Minimum of 2 years of experience in wound care or related field preferred.
Previous experience in a clinical setting focusing on advanced wound management.
Experience working in home health care, hospice, or palliative care settings is a plus.
