Job Responsibilities:

Provide comprehensive wound assessments and care plans for patients with complex wound care needs.
Implement advanced wound management protocols to ensure optimal healing outcomes.
Educate patients and their families about wound care and prevention strategies.
Collaborate with interdisciplinary teams to formulate and adapt care plans as needed.
Document patient care accurately in medical records in compliance with healthcare regulations.
Act as a resource for nursing staff on wound care best practices and interventions.
Participate in quality improvement initiatives aimed at enhancing patient care and outcomes.
Stay updated on the latest advancements in wound care management and treatment modalities.

Essential Qualifications:

Current and valid Registered Nurse (RN) license in the state of California.
Wound Care Certification (WCC) or similar accreditation is required.
Strong clinical assessment skills and proficiency in wound care management.
Excellent communication and interpersonal skills for effective patient interaction.
Ability to work collaboratively within an interdisciplinary healthcare team.

Desired Experience:

Minimum of 2 years of clinical experience in wound care or related settings.
Experience with advanced wound management techniques and patient education.
Prior experience in home health or hospice care settings is a plus.
Demonstrated commitment to continued professional development and education.
